FORMS! This is the place to find membership and event forms To apply for or renew your H.O.G. Chapter membership, download, print, read, fill out and sign all pages of the “Membership Application”. Dues are $20.00 per year and are due on January 1 of each year. They will be prorated for new members according to the month membership is received. Oregon No. 1 HOG Membership
c/o D&S Harley-Davidson 1341 Center Drive Unit C, Medford. OR. 97501 OR bring your forms to D&S. There is a chapter mailbox about 20 feet on the left just inside the double door entrance to D&S. You can drop your forms and payment in the mailbox. Our membership officer will get it and process it for you.
